We always welcome new clients! When calling our clinic for the
first time, you will be asked a number of questions. Our receptionists will ask
your name, address, home, work, and cell phone numbers, animal's name, breed,
color, date of birth, sex, vaccination history, reason for visit and preferred
method of payment. They may ask additional questions about your pet's health and
symptoms . If your first appointment with us will be in our clinic, you will be
asked to complete a few simple forms when you arrive. Please bring
your pet's medical records, including vaccination history, with you. If your
first appointment is a farm call, the forms will be mailed to you with a
convenient return envelope provided for you. Please complete and return the
